Submission Deadline: Monday, February 12, 2024
These reports MUST be completed using the free Adobe Acrobat Reader software (version 9 or higher).
Please submit both forms to your departmental administrator by clicking on the 鈥渟ubmit to AA鈥� box at the top and bottom of each form. This will open an e-mail dialog box. In addition to electronically sending the Annual Report of External Consultation Activities, you must also print and sign it then forward it to your departmental administrator.
Please note聽these forms may not work with certain browsers or in preview mode. Users may聽need to download and save the .pdf files, then manually open them using Adobe Acrobat Reader.
