Medical Notes & Letters of Support

All medical notes or letters of support are written on a case-by-case basis and at a clinician's discretion. Clinicians do not make academic or accommodation decisions. The Student Wellness Hub cannot accommodate drop-ins or same-day appointments. Please look for same-day appointments here.

91社区's Policy on Medical Notes

  • Medical notes will 鈥痮nly鈥� be issued鈥� on the day鈥� of the missed exam, assignment, project, class or conference. Absolutely 鈥痭o 鈥痬edical notes will be written for past illness (i.e. 鈥痮n a weekend, the day before, etc.).鈥�

  • Medical professionals 鈥痙o not 鈥痟ave the authority to exempt students鈥� from exams, presentations, etc. The final decision will be taken by your professor, faculty and/or department.鈥�

  • Because of the high volume of patients, please鈥� call as early as possible 鈥痶o book an appointment and ensure you鈥檒l be seen as soon as possible.

As we cannot accommodate drop-ins, please consult鈥� 鈥� for a list of off-campus clinics. Please note you may鈥� be charged for this service.

From Counsellor or Local Wellness Advisor

While counsellors and local wellness advisors don't make decisions regarding academic accommodations, they can provide letters of support in certain cases.

If you see a clinician before exams or deadlines, you can talk to them about receiving a letter outlining the circumstances that have contributed to any academic difficulties.

Letters are not written after the fact鈥� if the clinician has no previous knowledge of the circumstances leading up to the difficulty. Documentation is only provided at follow-up appointments with a clinician (not an initial consultation).

From a Psychiatrist

Medical notes from a psychiatrist are written on a case-by-case basis.

From a Doctor

You can request a medical note during the appointment with a doctor.鈥疦urses cannot give medical notes.
